I have 34 years old male with hx of schizoaffective disorder, anxiety, ptsd and depression. I had asked questions about this pt earlier and got good response. I saw him not long ago and he had this restlessness I could not explain also feelings of impending doom. Says his compliant with meds. Attempted to get a collateral but could not. He is on lithium, olanzapine, Abilify, clonazepam, hydroxyzine, lexapro, gabapentin. Am in the process of weaning the benzos and given instability especially anxiety I have not adjusted his medication. His lithium level is subtherapeutic, 0.3, he is on 900mg I increased to 1200, increasingly restless when I saw him. Does Akathisia occur even when they are already on benzos since thats one of the treatment? What medication would you adjust to help with symptoms? He smokes Marijuana which he gets from the dispensary. His olanzapine and abilify have a lot of room for increase,didn't want to increase it due to side effects. Will draw lithium level 7 days after increasing lithium.?
